PAPER RENEWAL FORM

All PA licenses expire on December 31st.  We encourage you to complete the online renewal or complete and return the paper form and the $10 renewal fee to the Board office as soon as possible. However, to ensure you are not penalized by the delay in our opening PA license renewals, the Board of Medicine has suspended all late renewal fees and paper form processing fees.
